HOME CARE SERVICES Our Home Care professionals specialize in customized services for your aging loves ones.
History
You know the Easter Seals name because we have cared for your children, sister or brother. Now we are expanding our home-care services to meet the needs of your parents and grandparents, as well as our veterans. For over 60 years, Easter Seals has been a leader in the provision of services geared toward helping individuals with disabilities or special needs achieve full community integration and inclusion enabling them to remain in their homes for as long as possible while living an independent lifestyle. Easter Seals Home Care Services offers one-stop shopping; we will meet you at your home, place of business or family member’s home to discuss your needs.
Services
Geriatric Care Management: Geriatric Care Managers are health and human services specialists who help families care for older relatives, while encouraging as much independence as possible. They may be trained in any of a number of fields related to long-term care, including, but not limited to, nursing, gerontology, social work, or psychology, with a specialized focus on issues related to aging and elder care. The Care Manager acts as a guide and advocate -- identifying problems and offering solutions, from assessment of an aging parent's needs to addressing the life change of a family affected by Alzheimer's Disease, Parkinson’s or other symptoms of dementia. Assessment and Development of Plan of Care: During the assessment we cover the client’s present living situation, and yours and your loved one’s goals, we assess cognitive, nutritional, social and safety needs. Our RN will design an individualized Care Plan and implementation process.
Long Distance Care Management: In today’s world, it is common for family members to live in different cities and states. Everything is fine, and then….. a call from one parent or the hospital, or police….. Mom or Dad has fallen and broken a hip; is confused during your phone check-in. Our Care Managers connects older Americans and their caregivers with sources of information on senior services. Our service links those who need assistance with state and local area agencies on aging and community-based organizations that serve older adults and their caregivers. We will also arrange home care services for mom or dad. Our Care Manager is on the scene to advocate for the family.
NonMedical In Home Care: Services are based on the individualized assessment and the plan of care. Services provided by our certified staff include but are not limited to:
· Assistance with activities of daily living · Showering, dressing, grooming · Toileting · Meal preparation · Light housekeeping · Yard work · Socialization · Organizing insurance documents · Shopping
Our home health aides provide care under the direction of our RN. After the initial assessment our RN will visit every 60 days and as needed to review your needs.
Companionship: Our companions will help relieve loneliness by encouraging conversation, reading, and other activities suited to each client’s interest.
Sitters in Hospitals or Nursing Facility: Provides personal care needs during hospital stays and watches over the patient in the absence of family and friends.
Benefit Counseling and Document Preparation: Assistance with navigating the maze of benefits and needs for long term care. From help with VA benefits, choosing assisting living facilities, preparing Medicaid documents, or advanced directives, our Care Managers are here to help.
Technology: 95% of seniors want to remain independent and living in their own home, this can often be a challenge. We offer access to a wide range of technology designed to assist the senior to live safely in their homes. Please call to discuss your loved ones circumstances and for a home safety evaluation.
Caregivers / Staff
Our staff of caregivers are compassionate, trustworthy, patient, and educated. Each caregiver is certified in their specific area of care giving and is able to provide qualified care for your loved one. Our staff members go through a rigorous process to determine their eligibility for employment. Easter Seals conducts criminal background checks, reference checks and fingerprinting. Each caregiver is required to attend 12 hours of annual training. All Caregivers are direct employees of Easter Seals New Jersey.
Areas of Service
Our services are presently offered in Monmouth and Northern Ocean Counties. All services are tailored to meet your loved ones needs. The family, client and Easter Seals Care Manager determine the caregiver’s schedule. We offer 24/7 care with a (2 hour minimum; see services above).
Speaker Services
Would you like a speaker for your group or organization? Topics include how to choose an assisted living, accessing veteran’s benefits, Medicare Part D. Choosing the right drug provider, preparing advanced directives and document preparation for a Medicaid application. We can also arrange for an Elder Care Attorney to speak with your group.
How Much Does the Service Cost?
Services are billed per-hour. The hours required to properly care for your loved one will be based on their needs assessed on the interview and on an ongoing basis.
